The Patient Navigator program is an enhanced medical escort service offered free of charge to adults aged 60+ who live in Framingham. The program is designed to provide support to older adults so they can get the medical care and information they need to live safely and independently in the community.
JFS is proud to partner with the MutualOne Charitable Foundation and is grateful for their support of the Patient Navigator program.
