Job Title: Case Manager in JFS Family Assistance

This particular position is centered in JFS’ body of work in the Jewish community.
The JFS Family Assistance Network’s goals are to help relieve financial distress of Jewish individuals and families and to support them in their efforts to reach financial stability.
In order to accomplish these goals, JFS’ Family Assistance Network serves as the primary point of intake and support for Jewish families and individuals in or at risk of poverty throughout the MetroWest’s communities. The overarching goal of the program is to move families from crisis to longer range stability.

Position and Requirements:
The Case Manager will have responsibilities across the three primary areas of JFS Family Assistance work:

  1. The family assistance program’s core focus is upon basic need services to Jewish families in distress and their financial stabilization (approximately 60% of one’s total time).
  2. Ready for Success, JFS’ pilot mid/late career re-employment program (15-20% of time).
  3. Geriatric care management and its short term assessment and problem solving component for subsidized and free care clients (15-20% of time).

The Case Manager will be responsible for the initial assessment of need, follow-up services and referrals and ongoing case management as necessary. On an ongoing basis, the case manager will have the following duties:

  • Initial intake and assessment of client need.
  • Assess the urgency of the need based on phone or face to face intake.
  • Complete the Client Assessment Indicator to determine level of vulnerability and prioritize actions needed to move towards stabilization.
  • Complete the initial client screening and enter all necessary information into the database in real time.
  • Provide emergency assistance based on assessed basic needs. This includes limited cash assistance and access to food, utilities, transportation and housing.
  • Provide and facilitate referrals to services offered by partner agencies and local and state agencies.
  • Give/send the client the program application and request completion within two weeks.
  • Review application for completeness, request additional information if needed and determine eligibility based upon program requirements.
  • Develop Action Plan with each client based upon Assessment Indicators and follow up with each client to ensure that appropriate steps have been taken and connections to resources have been made. Modify Action Plan as needed.
  • Track movement through the use of client assessment and planning tools.
  • Provide similar case management support as required for those in the employment program, as well as work with volunteer mentors and related supports.
  • Provide short term geriatric care management assessment and problem-solving services to a limited number of subsidized and free care cases.
  • Participate in partnerships and coordination with Combined Jewish Philanthropies and the system of human/social service providers.
  • Track and complete all performance measures as required to accurately measure program outputs and outcomes.
  • Maintain the database of all client activities.

  • Ability to analyze data and create reports.
